Health and Wellness Director Position Summary: -
The Health and Wellness Director works as a liaison between residents, resident families, and staff. The Health and Wellness Director duties also include problem-solving resident concerns and coordinating care with the Caregivers. The Health and Wellness Director may be an RN or LVN when necessary, as well has have supervision over licensed nurses and care staff.

    -Responsibilities of the Health and Wellness Director:

    • Responsible for the direct supervision of licensed nursing staff (LPN/LVN, RN). Med Techs/CMA/CNAS (if required per State Regulations) and all Care staff. May also supervise Resident Care Coordinator.
    • Coordinate admissions with assistant administrator including supervising move-ins to be sure -accommodations are as desired and care is immediately implemented.
    • Complete family/resident admission interviews.
    • Immediately bring prohibited conditions or at-risk residents in need of a higher level of care to the -attention of the assistant administrator.
    • Coordinate care planning with home health agencies on site, working within community policy.
    • Arrange for transportation as desired by the resident.
    • Arrange for resident special needs involving other departments, verifying follow through.
    • Function as a liaison with families ensuring special needs/requests/complaints are addressed. Inform -assistant administrator, in writing and verbally, of all family or resident complaints.
    • Monitor staff performance, providing or arranging assistance as needed.
    • Read all communication notes regarding the community between the Caregiver shifts.
    • Coordinate staff training and in-service schedules with the Assistant Administrator
    • Supervise the medication room and orders, working with and supervising Medication Aides and Caregivers.
    • Other duties as assigned. -

      Education and Experience: -

      • Bachelors Degree in Health Care or related field from an accredited college preferred, may require education to obtain state nursing license (LVN/LPN or RN) per State Regulations,
      • A minimum of three year's relevant experience.
      • Basic typing skills along with a working knowledge of personal computing and word processing software are required, preferably in a Microsoft Windows environment. -
