Description:$145 - $230 per visitJob SummaryAmerican Hospice and Home Health Services is currently seeking a Full Time, Part Time or Per Diem service either one or a combination of these counties:West Contra CostaAmerican Hospice and Home Health Care is seeking dynamic and highly motivated Clinicians to join its team. We currently provide Home Health patient care in Alameda, Contra Costa, San Mateo, Santa Clara, San Joaquin, Solano, and Stanislaus Counties.As the Home Health Physical Therapist, you will provide physical therapy services to patients according to a written physician's plan of care. This will include assessing and evaluating patients and their home environment, assesses for muscle strength, mobility, gait, ROM. As well as participating in the development of the total plan of care Supervises and overseeing the Physical Therapy Assistants according to organization policy.Requirements:Responsibilities and DutiesAbility to perform evaluation in the home and develop a care plan.Help patients with mobility, movement, and help regain their independence.Devise detailed treatment and care plans for each of the patients.Ability to complete all documentation in accordance with policy and procedure.Flexible, as varied daily activities will include phone, email and written communication, meetings with a variety of healthcare professionals, great understanding of hospice rules and regulationsMust work closely with team members to provide the optimal care and services.Ability to manage scheduling visits, documentation time, and meetings effectively.Ability to learn an electronic medical records system.Qualifications and SkillsLicensed Physical Therapist in California2 years' experience as a physical therapist preferred in Home HealthCurrent CPR CertificationAbility to drive within 30-80 miles a dayAbility to pass background screeningsAbility to function autonomouslyStrong clinical skills in timely medical documentationEffective communication skillsProficient computer skillsReliable transportation and insuranceWorkplace Setting:Home VisitThis Job Is Ideal for Someone Who Is:Dependable -- more reliable than spontaneousPeople-oriented -- enjoys interacting with people and working on group projectsAdaptable/flexible -- enjoys doing work that requires frequent shifts in directionCompensation details: 60-75 Hourly WagePIc9879271e99c-25660-37423650
