Registered Nurse (RN) | Home Health Location: Alameda, CA Agency: Cynet Health Pay: $2,563 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/23/2026 About the Position Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 5 years License: Valid RN License Certifications: AHA BLS Must-Have: - Previous Charge Experience - Epic Charting System Experience Description: Under the supervision of the Clinical Manager, the Team Nurse is responsible for delivering appropriate, adequate, effective, and efficient direct patient care services assigned by the Primary Care Clinician and approved by the Clinical Manager. Responsibilities include the implementation of the physician plan of care, obtaining physician orders, supervision of home health aides, and coordination of care within an interdisciplinary team. Participation in effective utilization management and control of patient supplies is essential. The Team Nurse consistently provides care based on the age-related needs of the unique population, considering their developmental and psychosocial needs, environmental safety, educational needs, communication style, and level of understanding. The Team Nurse is accountable to the Primary Care Clinician for ensuring that the plan of care is implemented following professional standards of care and agency-specific protocols.
