Crisis Coverage Schedule: Day Shift (typically 9:00 AM – 5:00 PM)
Setting: Skilled Nursing Facility (Subacute & Long-Term Care)
Position Summary The Speech Therapist (SLP) provides comprehensive
assessment, diagnosis, and treatment of speech, language,
cognitive-communication, and swallowing (dysphagia) disorders for a
geriatric, medically complex population in a subacute and long-term
care SNF setting. This role collaborates closely with the
interdisciplinary team to support quality patient outcomes and
regulatory compliance. Key Responsibilities - Conduct comprehensive
evaluations within 24 hours of physician referral to assess speech,
language, cognition, and swallowing function; establish functional
diagnoses and prognoses. - Develop, implement, and modify
individualized treatment plans using evidence-based interventions
with clear, measurable goals. - Provide direct therapy services
addressing communication and swallowing disorders, including
compensatory strategies, cognitive-communication therapy, and group
therapy as appropriate. - Complete accurate and timely
documentation, including evaluations, daily treatment notes, weekly
progress notes, 14-day physician reports, discharge summaries,
billing records, and plans of care. - Educate residents, families,
and nursing/CNA staff on communication strategies, safe swallowing
techniques, and home exercise programs. - Collaborate with the
interdisciplinary team (nursing, physicians, PT/OT, dietary, social
work) to optimize patient outcomes and support discharge planning.
- Participate in quality improvement initiatives and ensure
compliance with Medicare/Medicaid requirements, including PDPM,
CMS, and California Title 22 regulations. - If supervising SLPAs or
therapy aides, ensure compliance with California supervision
regulations, including required experience, supervision
percentages, and continuing education. Qualifications - Master’s
degree in Speech-Language Pathology from an accredited program. -
Active California SLP license (Temporary/RPE license accepted if in
process). - ASHA Certificate of Clinical Competence (CCC-SLP)
preferred. - Experience in a Skilled Nursing Facility, subacute, or
geriatric rehabilitation setting preferred. - Strong clinical
assessment, treatment planning, documentation, and communication
skills. - Knowledge of Medicare/Medicaid billing, PDPM, and
California regulatory requirements. - Current BLS/CPR certification
required. Work Environment & Expectations - Caseload includes
subacute rehabilitation residents (often Medicare Part A stays) and
long-term care residents. - Therapy intensity may reach up to
approximately 3 hours per day, depending on patient tolerance. -
High productivity expectations are common (often 80–90% billable
time), requiring efficiency in documentation and scheduling. -
Workload may fluctuate with admissions and discharges; flexibility
and adaptability are essential. - Strong collaboration with nursing
and CNA staff is critical to ensure coordinated, patient-centered
