Job Description:
Job Summary: The WelbeHealth Occupational Therapist is
responsible for coordinating and providing restorative and
rehabilitative Occupational Therapy services to participants at the
Center and at home. Occupational Therapy works closely with PACE
Physician, rehabilitation staff and other interdisciplinary team
(IDT) members to maximize participant independence and safety as
well as enhance performance of ADLs. The Occupational Therapist
collaborates with the Physical Therapist to efficiently operate
WelbeHealth rehabilitation department and is a member of the
WelbeHealth management team and facilitates collaborative
relationships across WelbeHealth and with external stakeholders to
promote the WelbeHealth values, team culture and mission.
Conduct Occupational Therapy assessments to determine the
Occupational Therapy care needs, preferences and goals of the
participants and actively participate in IDT meetings to develop
participant care plans.
Deliver and document Occupational Therapy interventions in
accordance with physician orders and/or as agreed upon in the
participants' care plans including but not limited to the use of
devices, techniques and activities to promote muscle re-education,
perceptual-motor and cognitive function, balance, endurance and
other factors affecting the level of independence in performing
ADLs.
Supervises the Therapy Assistants including providing training,
instruction and guidance needed to implement Functional Maintenance
Programs (FMP).
Provide training and instructions to staff, participants and
caregivers/families in areas of Occupational Therapy in which they
may work with participants.
Order, track and manage maintenance for equipment.

Manage direct reports including training, supervising and
mentoring.
Follow WelbeHealth policies and procedures and participate in any
required Quality Improvement activities, staff training and
meetings.
Timely completion of all mandated trainings and education.
Timely completion of all mandated occupational health screenings as
needed.
Minimum of two years Occupational Therapy experience in a clinical
setting with a frail or elderly population.
Occupational Therapy knowledge and skills necessary to treat frail,
elderly participants with complex needs.
Excellent leadership, organizational and communication skills in
settings with seniors, their families and interdisciplinary team
members.
Experience leading in a data-driven organization, leveraging
reports and data to prioritize and manage people and projects.
Excellent clinical, organizational and communication skills in
settings with seniors, their families and interdisciplinary team
members.
Ability to work independently with minimal supervision.
